Productivity Expectations
- Minimum 6 points per day required
- 32 hours/week: 24 points
- 40 hours/week: 30 points
- Points are required based on hours worked
Job Summary
We are seeking a compassionate and clinically skilled Registered Nurse (RN) to provide high‑quality home health services to adult patients, primarily serving an elderly population with chronic and acute health conditions. This role involves comprehensive clinical assessments, advanced skilled nursing care, patient education, and care coordination in the home setting. The ideal candidate demonstrates strong clinical judgment, independence, and exceptional patient-centered care delivery.
Key Responsibilities
- Perform comprehensive OASIS assessments, including Start of Care (SOC), resumption, recertification, and discharge visits.
- Deliver skilled nursing care to home health patients, including:
- Advanced wound care
- IV therapy and medication administration
- Management of PICC lines, central lines, and ports
- Ostomy care and catheter insertions/changes
- Basic medical-surgical nursing care
- Monitor and manage patients with chronic and acute disease processes.
- Assess patient conditions, identify changes in status, and communicate findings promptly to physicians and care team members.
- Educate patients and caregivers on disease processes, medications, treatments, and self-care techniques to promote safety and independence.
- Accurately document all clinical visits, assessments, and outcomes in accordance with regulatory and organizational standards.
- Coordinate care with physicians, therapists, and interdisciplinary team members to ensure continuity of care.
- Comply with infection control, safety protocols, and quality improvement standards.
- Travel daily to patient homes within the service area and between patient visits as part of regular duties.
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license in the state of Arkansas.
- CPR certification (current).
- Ability to work independently in a field-based role.
- Valid driver’s license with reliable transportation.
Preferred Qualifications
- OASIS experience.
- Prior home health nursing experience.
- Strong clinical documentation skills.
- Experience working with geriatric patients.
