Job Title: Home Health Registered Nurse (RN)
Position Overview
We are seeking a skilled Home Health RN to provide patient-centered care in a field-based setting, including assessments, care planning, and clinical interventions.
Key Responsibilities
- Perform Start of Care (SOC), recertification, and discharge visits
- Complete OASIS documentation accurately and timely
- Conduct routine skilled nursing visits and patient assessments
- Provide wound care, catheter care, and medication management
- Educate patients and caregivers on disease management and care plans
- Coordinate care with providers and interdisciplinary teams
- Maintain same-day EMR documentation
Required Qualifications
- Active Registered Nurse (RN) license
- Home health experience required
- Strong clinical, assessment, and patient education skills
- Ability to work independently in a field setting
- Willingness to participate in on-call rotations
- Must pass occupational health screenings
