Nurse Practitioner – HouseCalls

Overview

The Client is seeking qualified Nurse Practitioners (NPs) to conduct in-home health assessments for Medicare Advantage members. This role focuses on preventive care, identifying gaps in care, and supporting improved health outcomes through comprehensive, patient-centered assessments.

This is a field-based, pay-per-assessment position with flexible scheduling across a 7-day work week.

Key Responsibilities

Clinical Duties

  • Perform in-home health assessments (45–90 minutes per visit), including:
    • Medical history review
    • Review of systems
    • Physical examination
    • Medication reconciliation
    • Depression screening
  • Conduct clinical procedures as appropriate:
    • Vital signs collection
    • Monofilament testing
    • Urine dipstick testing
    • Foot exams
  • Identify and document current and historical diagnoses
  • Recognize care gaps and communicate findings to Primary Care Providers (PCPs)
  • Initiate appropriate referrals and recommend follow-up care
  • Identify urgent/emergent conditions and escalate as needed
  • Provide patient education on disease management and medication adherence
Documentation & Compliance Requirements

  • Complete and sync charts within 48 hours of visit
  • Complete within required timelines:
    • Collaborative physician reviews: within 3 days
    • Documentation discrepancies: within 7 days
    • Addendums: within 7 days
  • Maintain strict compliance with HIPAA and PHI standards
  • Assessments may be returned for correction if incomplete or inaccurate
Work Schedule

  • Hours: Monday–Sunday, 8:00 AM – 8:00 PM
  • Weekend coverage is permitted and encouraged
  • Candidates must maintain 6 weeks of availability in scheduling system at all times

Compensation

  • Paid per completed assessment
  • No mileage reimbursement
  • Please include an additional 5% in GM calculator

Minimum Qualifications

Licensure & Certification

  • Active, unrestricted NP license (no disciplinary restrictions)
  • National Board Certification:
    • Accepted:
      • American Academy of Nurse Practitioners (AANP)
      • American Nurses Credentialing Center (ANCC)
    • Not Accepted:
      • Mental Health
      • Pediatric
  • Basic Life Support (BLS/CPR) required (ACLS not accepted)
Autonomous Practice (State-Specific Requirement)

NPs must hold autonomous practice licensure in the following states:

  • CT, FL, IL, MN, NE, NY, SD, VA, VT

Accepted NP Specialties

  • Adult Gerontology Primary Care
  • Adult Nurse Practitioner
  • Family Nurse Practitioner
  • Gerontology Nurse Practitioner
  • Adult Gerontology Acute Care (ANCC, AANP, or AACN approved)

Acute Care-Only Certification Restrictions

Candidates with only Acute Care certification are eligible only in the following states: AK, AZ, CO, DE, DC, HI, ID, IA, KS, KY, ME, MD, MA, MI, MO, MT, NV, NH, NJ, NM, ND, OK, OR, RI, UT, VT, WA, WV, WY

Experience Requirements

  • Minimum 6 months of experience as a Nurse Practitioner
  • New graduates will not be considered
  • Preferred experience:
    • Home health or home visits
    • Geriatrics, cardiology, internal medicine, or endocrinology
Pre-Employment & Onboarding Requirements

Candidates Must Complete The Following Prior To Training

  • Background check, drug screening, and TB test
  • Application and credentialing documentation submission
  • Completion of all pre-requisite training by the Monday before start date
  • Training date cannot be changed once selected
  • Completion of mock assessments and learning modules within 7 days of orientation

Required Documentation

  • Active license verification
  • Board certification verification
  • BLS/CPR certification
  • NPI number
  • Confirmed home address (for equipment shipment)

All candidates must be approved prior to offer.

Operational Expectations

  • Candidates must:
    • Call and confirm visits with members the day prior (unpaid)
    • Be prepared for diverse home environments:
      • Urban, rural, apartments, group homes, and more
    • Handle environmental factors such as pets, smoking, and other conditions
  • Driving is required:
    • Must have reliable transportation
    • Ability to navigate varying road and weather conditions
    • Manage parking challenges
Safety Protocols

  • Dedicated safety monitoring for high-risk areas
  • Optional use of safety tools (e.g., mobile safety apps)
  • Geo-fencing and restricted area guidelines
  • Escalation protocols for unsafe locations

Skills & Competencies

  • Strong communication skills (especially with geriatric populations)
  • High attention to detail and organizational skills
  • Ability to work independently
  • Proficiency with EMR systems and technology
  • Physical ability to:
    • Lift up to 30 lbs
    • Navigate stairs and varied home environments
    • Perform physical assessments
Interview Process

  • Interviews are conducted exclusively by Judge Group
  • Client does not conduct interviews
  • Candidate must complete attestation form prior to consideration

Additional Notes

  • “No Show” is defined as a patient canceling after NP is en route or on-site
  • Candidates should review Vendor “Need to Know” materials prior to start
