Position Details
- Position: Registered Nurse (RN) - Home Health
- Coverage Area: Covington, Conecuh, and Butler Counties, AL
- Schedule: Monday - Friday, 8:00 AM - 4:30 PM
- Weekly Hour Guarantee: 40 Hours
- Productivity Expectation: 30 Points
- On-Call Requirement: Yes
- Weekend Requirement: Yes
- Mileage Reimbursement: Yes, between patient visits
- Provide skilled nursing care to patients in their homes in accordance with physician orders and agency protocols.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Monitor patient conditions and document all care provided accurately and timely.
- Educate patients and caregivers regarding disease management, medications, treatments, and overall health maintenance.
- Coordinate care with physicians, therapists, social workers, and other members of the healthcare team.
- Perform wound care, medication management, and other skilled nursing interventions as required.
- Maintain compliance with state, federal, and agency regulations.
- Complete all required documentation using Homecare Homebase (HCHB).
- Active Registered Nurse (RN) license in the State of Alabama.
- Current BLS/CPR certification.
- Previous Home Health nursing experience.
- Experience with Homecare Homebase (HCHB) documentation.
- Valid driver's license, reliable transportation, and current vehicle insurance.
- Strong clinical assessment and critical thinking skills.
- Ability to work independently and manage a patient caseload efficiently.

- IV site care and maintenance.
