Travel Registered Nurse (RN) - Home Health

Location: Elizabethtown, KY

Schedule: Monday-Friday, 8:00 AM - 4:30 PM EST

Hours: 40 hours per week

Productivity Expectation: 30 productivity points per week

On-Call Requirement: 7-10 days per month, including one weekend (Friday, Saturday, and Sunday) per month

Position Summary

We are seeking an experienced and compassionate Registered Nurse (RN) to join a Home Health team serving patients throughout the Elizabethtown, Kentucky area. The RN will provide skilled nursing services in the home setting, complete comprehensive patient assessments, coordinate care with the interdisciplinary team, and ensure high-quality patient outcomes through accurate documentation and effective clinical decision-making.

The ideal candidate is proficient in OASIS documentation, excels in patient-centered care, and possesses strong clinical judgment and communication skills.

Essential Responsibilities

• Complete Start of Care (SOC), Recertification, Resumption of Care (ROC), Discharge, and other OASIS-required assessments.
• Develop, implement, and update individualized patient care plans based on comprehensive assessments.
• Provide skilled nursing care and patient education in accordance with physician orders and agency policies.
• Ensure timely, accurate, and compliant documentation of all patient visits and clinical activities.
• Monitor patient progress and identify changes in condition requiring intervention or escalation.
• Communicate effectively with physicians, patients, caregivers, and interdisciplinary team members regarding patient care needs and treatment plans.
• Demonstrate strong customer service by building positive relationships with patients, families, referral sources, and team members.
• Promote patient safety through ongoing assessment and identification of potential risks within the home environment.
• Participate in on-call coverage as assigned and respond appropriately to patient needs.
• Collaborate with the Executive Director, Patient Care Manager, providers, and other healthcare professionals to ensure continuity of care.
• Adhere to all federal, state, agency, and accreditation standards related to home health services.
• Perform additional duties as assigned.
  
  

Coverage Area

Travel throughout the assigned service area to provide home health nursing services. Candidates must be comfortable working independently in various home care settings.

Mileage Reimbursement

• Mileage reimbursement is provided between patient visits, beginning after the first patient visit of the day and ending at the last patient visit of the day.
• Mileage is not reimbursed from home to the first patient visit or from the last patient visit back home.
  
  

Required Qualifications

• Active Registered Nurse (RN) license in the State of Kentucky.
• Minimum of one (1) year of Registered Nurse experience.
• Current BLS certification through the American Heart Association (AHA).
• Valid driver's license.
• Current automobile insurance.
• Reliable transportation for patient visits.
