We are seeking an experienced and compassionate Physical Therapist (PT) to provide skilled home health services throughout Hardin, Meade, and Breckinridge Counties. The Physical Therapist is responsible for evaluating patients, developing individualized treatment plans, implementing therapeutic interventions, and monitoring patient progress to improve functional mobility, reduce pain, and enhance overall quality of life.

Essential Responsibilities
- Perform comprehensive patient assessments, including functional status, mobility, endurance, pain, and evaluation of all body systems required for CMS documentation.
- Develop and implement individualized physical therapy plans of care in collaboration with physicians and the healthcare team.
- Provide skilled physical therapy services in accordance with state practice guidelines, agency policies, and evidence-based standards of care.
- Monitor patient progress and communicate changes in condition to physicians, nursing staff, and other members of the care team.
- Deliver treatments designed to restore function, relieve pain, improve mobility, and maximize patient independence.
- Educate patients, family members, caregivers, and healthcare team members regarding treatment plans, progress, and functional limitations.
- Complete all required evaluations, documentation, progress notes, and care plans within established timelines.
- Report evaluation findings, treatment goals, and recommended visit frequencies to the supervising nurse within 24 hours of assessment.
- Travel throughout the assigned service area to provide patient care in home settings.
- Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as permitted by agency policy and state regulations.
- Participate in case conferences, staff meetings, continuing education, and quality improvement initiatives.
- Prepare discharge summaries and periodic patient status reports as required.
- Maintain current clinical knowledge, licensure, and competency standards.
- Perform additional duties as assigned.
- Mileage reimbursement is provided for travel between patient visits, beginning with the first patient and ending with the last patient of the day.
- Travel from home to the first patient and from the last patient back home is not reimbursed.
- Current Physical Therapist (PT) license in the State of Kentucky.
- Current CPR certification.
- Valid driver's license.
- Current automobile insurance.
- Reliable transportation for daily patient visits.
- Ability to work independently and manage a flexible patient schedule.
- Home Health experience.
- OASIS documentation proficiency.
- Strong clinical assessment and care planning skills.
- Excellent communication and interpersonal skills.
- Team-oriented approach with a positive attitude.
- Strong time management and organizational abilities.
