Schedule: Monday–Friday, 8:00 AM–4:30 PM

Weekly Guarantee: 32 hours

Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient

Pay Rate: $30-$32/hr

About The Role

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Home Health team serving patients throughout Limestone County, Alabama. This role is ideal for an LPN who thrives in an autonomous environment, enjoys one‑on‑one patient care, and values predictable weekday scheduling with no on‑call or weekend duties.

Key Responsibilities

• Provide skilled nursing care to patients in their homes under physician orders.
• Perform routine clinical tasks including medication reconciliation, wound care, vital signs monitoring, and comprehensive patient assessments.
• Accurately document all care provided in the electronic medical record (EMR).
• Educate patients and caregivers on disease management, medications, and individualized care plans.
• Communicate effectively with RNs, physicians, therapists, and the interdisciplinary care team to support coordinated care.
• Maintain compliance with state regulations, agency policies, and infection control standards
  
  

Required Qualifications

• Active Alabama LPN License
• Home Health experience
• Proficiency with Homecare Homebase (HCHB) EMR
