Our client is currently seeking a motivated and compassionate Travel Physical Therapy Assistant (PTA) to support home health services across Multnomah and Clackamas counties. In this role, you will provide high-quality rehabilitative care through patient visits, working closely with physical therapists and care teams to support patient recovery and mobility.

This position is ideal for candidates who thrive in independent, patient-centered environments and are comfortable working in a home health setting.

This Job Will Have The Following Responsibilities

• Perform subsequent patient visits under the direction of a licensed Physical Therapist
• Implement and support individualized treatment plans to improve patient mobility and functionality
• Monitor and document patient progress, reporting changes to the supervising Physical Therapist
• Educate patients and caregivers on exercises, safety techniques, and mobility practices
• Maintain accurate and timely documentation in compliance with clinical and regulatory standards
• Travel between patient homes within assigned coverage areas
• Collaborate with interdisciplinary care teams to ensure coordinated patient care
  
  

Qualifications & Requirements:

• Active Oregon Physical Therapy Assistant (PTA) license
• Prior home health (HH) experience required
• Current CPR certification
• Valid driver’s license and reliable transportation
