The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates clinical care for members in a facility setting. As an Advanced Practice Clinician (APC) within SCC, you will provide care to our highest-risk health plan members while working as part of an interdisciplinary team. This role offers true work-life balance with flexible scheduling, no on-call, no weekends, and no holidays required.

Primary Responsibilities
- Conduct comprehensive assessments of patients
- Manage acute and chronic medical and behavioral conditions
- Ensure accurate documentation including ICD-10 coding
- Collaborate with interdisciplinary care teams
- Lead advanced illness and care planning conversations
- Provide counseling and education to patients and caregivers
- Active and unrestricted license in your state (or ability to obtain by start date)
- Education:
- NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
- PA: Graduate of an accredited Physician Assistant degree program
- Certification:
- NP: Board certified through AANP or ANCC (Family NP, Adult NP, Gerontology NP, or Adult-Gerontology Acute Care NP)
- PA: Board certified by NCCPA
- Current, active DEA licensure/prescriptive authority
- 1 year of clinical experience (long-term care preferred)
- Experience with patients who have complex behavioral, social, or functional needs
- Knowledge of geriatrics and chronic illness management
- Experience with advanced illness and end-of-life discussions
- Proficiency with electronic medical records and documentation
