This role requires frequent travel within the assigned coverage area and active participation in interdisciplinary team collaboration to ensure optimal patient outcomes and quality end-of-life care.

Key Responsibilities
- Provide direct hospice nursing care to patients in home-based settings.
- Conduct comprehensive patient assessments and ongoing evaluations of physical, emotional, and psychosocial needs.
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
- Serve as the primary case manager for assigned hospice patients.
- Coordinate care with physicians, social workers, therapists, chaplains, and other healthcare professionals.
- Participate actively in Interdisciplinary Group (IDG) meetings and care planning discussions.
- Educate patients, families, and caregivers regarding disease progression, symptom management, medications, and hospice services.
- Document patient care accurately and timely using Homecare Homebase (HCHB).
- Monitor patient conditions and respond appropriately to changes in clinical status.
- Perform hospice admissions and comprehensive needs assessments as required.
- Maintain compliance with all regulatory, agency, and clinical practice standards.
- Active Registered Nurse (RN) license in good standing.
- Minimum Associate Degree in Nursing (ADN).
- Prior Home Health and/or Hospice nursing experience.
- Experience using Homecare Homebase (HCHB).
- Strong clinical assessment and case management skills.
- Excellent communication, interpersonal, and patient care skills.
- Ability to work independently and effectively manage a caseload.
- Valid driver's license, reliable transportation, and ability to travel throughout the assigned territory.
- Experience performing hospice admissions and hospice needs assessments.
- Strong end-of-life care knowledge and symptom management expertise.
- Exceptional bedside manner and relationship-building skills.
- Previous travel nursing experience in home health or hospice settings.
