This position is part of a team of health care professionals providing comprehensive support services to clients at the John G. Bartlett Specialty Practice, the adult outpatient HIV program at Johns Hopkins Hospital. We are seeking a Sr. Community Outreach Specialist. Under the direction of the Director of Social Work, this position will coordinate between three different sites to assist in critical information tracking of Ryan White Program eligibility, data collection, and linkage with community resources. The position is focused on supporting the role of the Case Management department in optimizing the Ryan White Eligibility Intake process and data collection, which improves Case Manager efficiency and effectiveness.

Specific Duties And Responsibilities
- Enroll Clients with HIV/AIDS from the Clinic in the Ryan White Program.
- Conduct Social Determinants of Need screening prior to a medical appointment.
- Address high-risk social determinants prior to medical appointment through referrals to case management, peer navigators, medical team collaboration, and/or community resources.
- Collect income information prior to the medical/case management visit, or prepare patients to bring in financial documents needed prior to the visit.
- Participate in the Ryan White eligibility committee to evaluate best practices for information collection in Baltimore City and Baltimore County offices.
- Develop a tracking system to ensure Ryan's eligibility stays current through updated financial and other psychosocial information.
- Documents client interactions in the hospital's electronic medical record system and provides updates of the client's public assistance application(s) status to the Case Manager.
- Accompanies clients to appointments at the Department of Social Services, Social Security Administration, Motor Vehicles Administration, and other community agencies to obtain records needed to apply for public benefits.
- Provides Case Managers with an updated list of HIV community resources.
- Updates educational materials for clients and staff, including pamphlets, posters, and brochures.
- Assist patients in improving their adherence to HIV care and other medical illnesses.
- Provide education on mental health and substance abuse programs.
- Assist patients in applying for health insurance and prescription programs such as ACA, MADAP, Medicare, Medical Assistance, Social Security Extra Help, QMB, SPDAP.
- Refer patients to the Department of Social Services for programs such as Food Stamps, TDAP, TCA, Eviction Prevention, and Energy Assistance.
- Provide education on the disability programs provided by the Social Security Administration, such as SSI and SSDI.
- Refer patients to community resources programs for housing/shelters, food services, and employment.
- Arrange patient's transportation services that include cabs, tokens, and referrals to the Moveable Feast transportation program.
- Assist patients in enrolling in transportation programs offered by the MTA and Medical Assistance.
- Performs other duties as assigned or required.
- Bachelor's Degree.
- Some related work experience.
- Additional education may substitute for required experience, and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula.
