What You’ll Do:
• Direct Care & Case Management: Provide direct and supportive health care to individuals in residential programs, assessing current health status and identifying needs.
• Health & Safety Oversight: Provide professional advice and direction on health and safety issues, ensuring the ongoing support of individuals’ health and medical needs.
• Collaborate with Treatment Team: Establish positive working relationships with individuals, families, and interdisciplinary team members, both within and outside the agency.
• Emergency Response: Initiate nursing interventions in emergency and non-emergency situations, adhering to established policies and clinical standards of care.
• Documentation & Care Plans: Establish and maintain nursing care plans, document individual health needs, and follow up on required actions.
• Appointment Scheduling: Ensure timely scheduling of medical appointments and services for individuals in your caseload.
• Staff Training: Provide training to staff on nursing care plans and agency in-services, ensuring proper care delivery.
• Medication Management: Oversee medication administration, ensuring proper labeling, storage, and disposal of expired medications according to policy and regulations.
• Hospitalization & Discharge Coordination: Communicate with hospital discharge planners and ensure safe discharges for individuals who have been hospitalized.
• Health Record Monitoring: Track and update health records, including seizures, bowel movements, vital signs, and other health metrics.
• After-Hours On-Call: Be available for after-hour on-call duties, responding to situations in accordance with policies and regulations.
• Safety Compliance: Follow safety guidelines to ensure a safe working environment and report any hazards.
• Program Coverage: May be required to float or cover other programs as needed or adjust work schedules to meet the needs of individuals.
Educational Requirements
• Bachelor’s or Master’s degree in Nursing, with at least one year of professional experience in delivering health care to individuals with Intellectual/Developmental Disabilities
•NYS RN Certification is mandatory
Experience & Skills:
• Valid unrestricted NYS Driver’s License and reliable transportation for travel between locations.
• Ability to triage urgent and crisis situations, problem-solve, and maintain focus in challenging situations.
• Must be flexible with days, hours, and location based on agency needs.
