This position plays a vital role in delivering personalized, client-centered nursing care that promotes independence, dignity, and quality of life through hands-on support, in-home assessments, and ongoing supervision. Serving as a key link between clients, families, and care teams, the role ensures safe, effective, and coordinated care while addressing concerns and improving outcomes. In addition, it provides leadership and mentorship to caregivers, fostering a culture of accountability and continuous improvement, while supporting compliance, quality assurance, and community-based programs to ensure seamless, reliable care that clients and families can trust.

JOB DESCRIPTION

Title: Registered Nurse

Location: 211 Smith Street, Sheridan, WY 82801

Reports to: Help at Home Clinical Manager

ESSENTIAL JOB FUNCTIONS

Client Care & Support

  • Promotes client independence, dignity, and quality of life through personalized, one-on-one support.
  • Ensures safe and effective nursing care, including medication management and personal care, to maintain client health and well-being.
  • Supports daily living and instrumental activities to help clients remain in their homes safely and comfortably.
  • Assesses and responds to individual client needs, resolving concerns to enhance satisfaction and outcomes.
  • Conducts in-home nursing assessments and supervisory visits to ensure adherence to care plans and optimal health outcomes.
  • Monitors and evaluates the performance of Day Break Certified Nursing Aides, ensuring high-quality care delivery.
  • Communicates client welfare promptly to care teams, facilitating coordinated, responsive care.

Compliance & Program Knowledge

  • Applies expertise in federal and state programs (Wyoming State Aging programs, Medicaid Community Choices Waivers, VA programs) to maximize client support and program effectiveness.
  • Maintains strict confidentiality and compliance with HIPAA and HITECH, protecting client trust and data security.
  • Conducts TB risk assessments and other required screenings to safeguard staff and client health.

Leadership & Organizational Values

  • Models the organization’s vision, mission, and cultural values to inspire staff and enhance team cohesion.
  • Provides supervision, mentorship, and coaching to homemakers and CNAs, improving staff performance and client outcomes.
  • Collaborates with Clinical Manager to deliver team development and training initiatives that strengthen workforce capabilities.
  • Supports the design and implementation of ongoing staff education to drive continuous improvement in service delivery.

Administrative & Other Responsibilities

  • Enhances community engagement by providing public relations support, including outreach, presentations, and educational materials.
  • Builds partnerships with community organizations to expand resources and support for clients.
  • Responds promptly to urgent client needs, ensuring timely interventions that prevent complications or disruptions in care.
  • Coordinates patient schedules based on physician orders and care plans, optimizing care delivery, and minimizing gaps.
  • Adjusts schedules and staffing in response to emergencies or caregiver absences, maintaining seamless service continuity.
  • Communicates effectively with caregivers, clients, and families to ensure expectations are met and concerns are resolved quickly.
  • Documents all interactions for transparency, follow-up, and quality improvement purposes.
  • Participates in Quality Assurance and Performance Improvement (QAPI) initiatives to enhance service quality, compliance, and client outcomes.
  • Completes additional tasks and responsibilities as needed to support organizational goals and operational success.

Knowledge, Skills, and Abilities

  • Coachable with a willingness to learn and adapt to a changing workflow.
  • Comprehensive understanding of patient care, including medication administration and chronic disease management.
  • Proficient in recording and managing medical information accurately and efficiently.
  • Teaching and coaching skills to educate patients and families about managing health conditions.
  • Organizational and time management skills to manage multiple tasks and prioritize effectively.
  • Ability to work collaboratively with healthcare professionals to ensure comprehensive patient care.
  • Demonstrated ability to provide compassionate, patient-centered care while maintaining professional boundaries.

Physical Demands

  • Access to reliable transportation for traveling between work locations.
  • Ability to move within a variety of settings, including patient homes and office spaces.
  • Occasionally lifting or moving items, such as medical supplies, or assisting patients with transferring
  • Adequate hand-eye coordination and fine motor skills to perform tasks such as medication administration and using medical tools or equipment.
  • Clear verbal communication for patient and team interaction, as well as the ability to document information accurately.
  • Ability to read medical charts, observe patient conditions, and listen effectively to verbal cues or instructions.

Minimum Qualifications

  • Licensed Nurse with a current license in the State of Wyoming.
  • Valid driver’s license with a safe driving record.
  • Ability to provide a clean, current Motor Vehicle Department (MVD) report.
  • Employment contingent upon passing a Central Registry background check.
  • Employment contingent upon passing a Division of Criminal Investigations background check.
  • One year of experience in a home health setting (preferred).
  • Experience working with older adults or people with disabilities (preferred).
