What You'll Do
T he Case Manager is responsible for developing and monitoring comprehensive participant service plans in collaboration with assigned participants. This includes, but is not necessarily limited to, internal and external referrals for services to stabilize medical and mental health conditions, barrier removal, job readiness coaching, job placement assistance, and retention services.
Your Responsibilities Will Include
- Initiate services for participants following their clinical assessment or clinical reassessment, providing orientation to services, explaining their rights and responsibilities, and answering questions.
- Develop and monitor wellness plans on behalf of participants with untreated or unstable medical and/or mental health conditions adversely impacting employability. This includes coordinating referrals to community-based medical and mental health providers.
- Regularly meet with assigned participants to monitor progress and compliance with wellness plans, as well as to assist with mitigating barriers to employment. These incudes, but is certainly not limited to housing, domestic violence, childcare, transportation, and reasonable accommodations.
- Conduct regular phone follow-up appointments with participants to assess progress toward wellness plan goals.
- Outreach to participants who have failed to comply with mandatory appointments and other program requirements to successfully re-engage them with their wellness services.
- Communicate with external medical and mental health providers to ensure participant compliance and timely completion of wellness plans.
- Provide intensive individual coaching for participants in job search activities and assist participants with issues pertaining to work-related and transitional benefits.
- Case conference with other WeCARE service providers on an as needed basis, including vocational evaluators, work readiness and computer instructors, job placement coordinators, job preparation counselors and WEP specialists.
- Document participant services and activities in Fedcap and HRA management information systems, including, but not limited to SEAMS and FedcapCARES .
- Bachelor’s degree in social work or related field, such as psychology, vocational rehabilitation, or occupational rehabilitation. MSW or master’s degree in a related field preferred.
- One (1) year of case management or related experience.
- Bilingual (English Spanish) preferred.
$23.08-$24.04/hr.
