What are the responsibilities and job description for the Social Service Specialist 1 - Referral Specialist position at The Children's Home Society Of New Jersey?
The Referral Specialist is for Monmouth and Ocean County. The CNJ Referral Specialist will provide referrals and resources to connect women and families to health care and other social service providers in the community. Responsibilities for this position include community outreach in both Monmouth and Ocean Counties coordinating with outreach to locate women/families without telephones to achieve contact for an initial assessment and medical care preconception.
Salary: $40,000 per year

Education and Experience Requirement:
The qualified applicant must be bilingual (English/Spanish) with a High School diploma, Associate's degree (preferred) and a minimum 1-year experience in maternal child health field and/or health related field. At least 1 year experience in community based public health field; excellent communication and problem solving and public speaking skills. Must demonstrate initiative, creativity, work well independently and as part of a team. This position requires travel in Monmouth and Ocean County.
- Attend all required meetings and training deemed necessary by DCF.
- Identify key contacts with local partner agencies and establish and maintain relationships, partnerships, and coalitions with community resources and services.
- Maintain ongoing relationships and conduct routine follow-up meetings with the prenatal and pediatric care providers.
- Support nurse home visitors as they connect families with community resources.
- Support and monitor the Post Visit Connection process to identify unmet community resource needs and/or referrals, customer satisfaction, and problem solve solutions for the community.
- Market the program by communicating with referral sources, groups that interact with families, and community leaders.
- Participate in relevant local and/or regional committees and task forces to communicate and collaborate regarding community perinatal and early childhood needs.
- Participate in joint community advisory board to assist in coordinating services within the early childhood system of care.
- Facilitate referral process and alignment of resources.
- Compile and maintain records, reports, and documentation of program activities regarding community relations for use in program evaluation.
- Facilitate one PDSA for each county per quarter.
- Participate in NJ Home Visitation (HV)/CNJ CQI/Evaluation Committee Meeting
- and evaluation activities.
Requirements:
- Experience with computer systems, data entry and word processing
- Knowledge of community resources in Monmouth and Ocean Counties
- Excellent written and verbal communication skills
- Ability to handle complex calls and situations
- Highly resourceful team-player, with the ability to also be extremely effective independently
- Proven ability to handle confidential information with discretion, be adaptable to various competing demands, and demonstrate the highest level of service and response.
