The Care Team is seeking a Licensed Practical Nurse (LPN) to join our hospice team in a 7-on/7-off on-call role. In this position, you will provide skilled nursing support to patients in their homes or facility settings under the direction of a Registered Nurse, in accordance with the Pennsylvania scope of practice. You will play a key role in after-hours care, supporting patient comfort, responding to urgent needs, and ensuring timely communication with the RN and interdisciplinary team.

In this position, you will provide skilled nursing support to patients in their homes or facility settings under the direction of a Registered Nurse, in accordance with the Pennsylvania scope of practice. You will play a key role in after-hours care, supporting patient comfort, responding to urgent needs, and ensuring timely communication with the RN and interdisciplinary team.
Schedule & Coverage
- 7 on / 7 off on-call schedule, working remotely, and dispatching in the field as needed.
- Weekday hours: 4:30 PM to 8:00 AM
- Weekend coverage: Friday 4:30 PM through Monday 8:00 AM
- Assigned coverage area based on branch needs and patient census

- Provide nursing care as delegated by the RN in accordance with the plan of care
- Respond to after-hours patient needs, including symptom support and urgent visits as directed
- Observe patient condition and promptly report changes to the RN
- Support comfort-focused care and reinforce education provided by the RN
- Document care and patient interactions accurately in Homecare Homebase (HCHB)
- Collaborate with the interdisciplinary team to ensure continuity of care
- Active LPN license
- Hospice experience preferred
- Experience using HCHB (Homecare Homebase) preferred
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
- Current TB testing in accordance with agency policy
- Current CPR certification as required for the role
