Schedule, Hours & Territory
- Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM
- Reading, PA and surrounding area
- Participation in an on-call rotation may be required, with additional compensation provided
- Meaningful role supporting patients and families emotionally and socially

- Conduct psychosocial assessments
- Provide counseling and emotional support to patients and families
- Assist with advanced care planning and coordination of community resources
- Participate in interdisciplinary team meetings
- Document services accurately and timely in the EMR HCHB (Homecare Homebase)
- Master’s degree in Social Work required
- Hospice experience preferred
- Experience using HCHB (Homecare Homebase) preferred
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
- Current TB testing in accordance with agency policy
- Current CPR certification as required for the role
