Responsibilities
- Complete OASIS Start of Care, Resumption of Care, Recerts, and Discharges
- Perform head-to-toe assessments and create individualized plans of care
- Teach patients and families on meds, safety, and disease management
- Coordinate with physicians and office clinicians for timely updates
- Document accurately in WellSky Kinnser and meet compliance standards
Qualifications
Required
- Active Texas RN license in good standing
- Minimum 1 year recent home health RN experience with OASIS
- Proficiency in WellSky Kinnser
- Reliable transportation and valid drivers license

Strong communication, organization, and time management
Preferred - BSN
- Bilingual Spanish or prior wound care experience
