License Practical Nurse (LPN)

Team Select Home Care is seeking compassionate and dedicated Licensed Practical Nurse (LPN) to provide 1:1 care for medically fragile patients in the comfort of their homes. Shifts typically range from 8–12 hours, and we offer flexible scheduling options to accommodate both full-time and part-time availability.

Our patients require support for a variety of medical needs. You’ll receive paid, hands-on training from our Director of Nursing to ensure you feel confident and prepared to provide the specialized care your patient needs.

We currently have shifts available in:

• Portland
• Gresham
• Canby
• Woodburn

Position Responsibilities

• Provide dedicated 1:1 patient care
• Work with both pediatric and adult patients
• Provide care including G-tube, tracheostomy, and/or ventilator support
• Complete electronic documentation accurately and timely
• Perform patient assessments, treatments, and other required care as outlined in the patient's individualized plan of care

Qualifications

• Active Oregon State LPN license
• Current CPR/BLS certification
• Current TB test
• Experience with trach, ventilator, and G-tube care is preferred but not required — paid training is available!
