Pay Rate:
LPN $27-$31 hourly
RN $35-$39 hourly
Location: Multiple Valley Locations
Schedule: Days & Nights Available | Flexible Scheduling
We are seeking compassionate and dependable Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our long-term care home health team. This role provides one-on-one, consistent patient assignments in the comfort of the patient’s home—offering a lower-stress alternative to hospitals and facilities while still delivering high-quality clinical care.

Responsibilities
- Provide skilled nursing care to pediatric and/or adult clients in the home
- Monitor and assess patient conditions
- Manage tracheostomies, ventilators, G-tubes, medication administration, and care plans (case dependent)
- Accurately document patient care and communicate changes to the care team
- Educate families and caregivers when appropriate
- Maintain patient safety and support quality of life
Qualifications
- Active Arizona RN or LPN license
- Current CPR/BLS certification
- AZ Fingerprint clearance card
- TB test (current)
- Experience with trach, vent, and G-tube care preferred — NOT required (training available)
- Strong clinical judgment and communication skills
- Reliable, compassionate, and patient-focused
Experience Level
