Overview: We are seeking an experienced Intake Coordinator to join our Intake team. The primary objective of this role is to ensure patients have a smooth transition of care from clinic/hospital to outpatient treatment by coordinating IV medication for patients in outpatient and/or home settings and organizing workflow and appointments for our IV infusion clinic.
Major Areas of Responsibility:
Completing intake referrals while coordinating with provider, pharmacy, and hospital case manager
Speaking with patients to ensure they understand their specific care plan
Specific Responsibilities of the Job:
Following up daily with patients and case managers for smooth discharge transition
Keeping orders current and accurate
Timely notifications to pharmacy/infusion nurses/insurance coordinator of new orders and changes to existing orders
Maintaining detailed notes/documentation on payment plans and conversations with patients
Documenting all pertinent patient information into eCW and Caretend
Maintaining flexibility and helping others as needed throughout the day
Communicating effectively with physicians, care coordinators, patients, and pharmacy staff
Documenting all orders, interventions, and conversations pertaining to patient care
Following all rules/expectations outlined in the employee handbook
Minimum Requirements:
High school degree or GED equivalent
Great computer skills and ability to learn new systems
Home Infusion experience
Experience with prior authorizations and benefits investigation
Payer knowledge (Medicare, Medicaid, Commercial)
Positive demeanor with patients and peers
Exceptional communication skills both written and verbal
Ability to work across departments
Patient-centered mindset
Work Environment:
Standard office environment within a healthcare clinic setting.
Regular interaction with healthcare providers, administrative staff, patients, and insurance companies.
Sitting for extended periods of time.
