A healthcare organization in Michigan is actively seeking a compassionate and dedicated Nurse Practitioner / Physician Assistant (NP / PA) to join their staff as their new Health Guide. In this role, the Health Guide (NP / PA) will assess both behavioral and physical health conditions through a thorough history and physical examination
Responsibilities
The Health Guide (NP / PA) will be responsible for:
- Capturing conditions not yet diagnosed, including by ordering additional studies as indicated
- Organizing the information in a manner that supports successful disability applications
- Conducting visits in the field or in an individual's home when needed
- Making individualized recommendations and/or referrals for programs/care
- Training and advising Guides and Community Resources Guides on clinical issues and treatments
- Supporting Guide Team on care coordination
- Leading care management
- Performing medication reconciliation post-discharge and as needed
- Communicating and coordinating with individual's care providers, primarily at CMHCs, FQHCs, and hospitals
Qualifications
- Active MI Nurse Practitioner
- 2 years clinical care experience as a Nurse Practitioner
- Experience working with and managing a population with challenges such as behavioral health and/or SUD conditions
- Experience developing and delivering educational programs to nurses and affiliated medical staff
- Care management and coordination experience
- Intermediate computer skills; familiarity with Microsoft Office Suite
