Requirements
- 3 years of hospital pharmacy experience in a high-acuity acute care setting
- Demonstrated experience in:
- NICU pharmacy practice
- Cardiac critical care (including Impella, ECMO, and advanced cardiac support cases)
- Active Arizona Pharmacist License (Required)
- Strong clinical judgment and ability to function independently in a fast-paced environment
- Experience with sterile compounding and critical care medication management
- Excellent communication and interdisciplinary collaboration skills
- Ensure safe, accurate, and timely dispensing of pharmaceutical agents to patients across all age groups
- Collaborate with physicians on medication selection, dosing optimization, and therapeutic alternatives
- Provide clinical consultation and medication education to nursing staff, physicians, and patients
- Support and participate in Medication Safety initiatives and quality improvement programs
- Monitor patient medication profiles to identify, prevent, and resolve drug-related problems
- Identify, document, and report adverse drug reactions in accordance with hospital policy
- Prepare and compound IV admixtures and parenteral nutrition (TPN) as required
- Maintain strict narcotic and controlled substance accountability per DEA regulations
- Ensure full compliance with state and federal laws, DEA requirements, TJC standards, and hospital policies
