Responsibilities
- Provide skilled RN home health visits to patients in their homes.
- Assess patient needs and develop individualized care plans.
- Monitor patient progress and adjust care plans as needed.
- Document all visits and interventions accurately in electronic health records.
- Meet or exceed established patient visit targets and productivity metrics.
- Coordinate multidisciplinary care and communicate with family/caregivers.
- Ensure compliance with regulatory standards and safety guidelines.
- Educate patients and families on disease management and self-care techniques.
- Document and report any adverse events or safety concerns promptly.
- Utilize data and pipelines to forecast patient needs and service depth in territory.
- Participate in continuous professional development and clinical skill enhancement.
- Collaborate with sales/marketing teams to communicate service value and expand reach.
- Current RN license in good standing with state board.
- Compliance with regulatory standards, safety guidelines, and HIPAA.
- Home health nursing experience preferred or documented proficiency in skilled visits.
- Ability to develop and adjust individualized care plans for home patients.
- Meet/exceed target visit counts and productivity metrics.
- Proven documentation accuracy in electronic health records (EHR).
- Strong collaboration with multidisciplinary teams and families/caregivers.
- Capable to perform adverse event reporting and risk assessment.
- Evidence-based patient education on disease management and self-care.
- Ability to forecast patient needs using data/pipelines; territory planning.
- Willingness to participate in ongoing professional development and clinical skill growth.
- Strong self-motivation with accountability for outcomes in a team-driven environment.
- Contract employment alignment; on-site visit expectations and travel readiness as needed.
