Responsibilities
- Provide skilled nursing care and monitor patient status per LPN LTC standards.
- Administer medications and document administration accurately in EMR.
- Assess patient needs, update care plans, and communicate changes to the team.
- Monitor and report vital signs, symptoms, and deviations promptly.
- Document all care and interventions clearly to ensure quality and compliance.
- Assist with daily living activities and maintain residents' comfort and safety.
- Collaborate with physicians, RNs, and care staff to optimize outcomes.
- Adhere to regulatory and facility policies; participate in quality improvement efforts.
- Maintain high productivity while achieving targets and meeting performance metrics.
- Identify opportunities for professional growth and participate in development programs.
- Assist with admissions, discharges, and transition of residents between units.
- Licensed Practical Nurse (LPN) in good standing with applicable LTC regulations.
- Provide skilled nursing care and monitor resident status per LTC standards.

- Maintain thorough, compliant documentation of all care interventions.
