Certification Requirements
- CA RN License
- BLS Certification
- 5 years of RN Case Management experience
- Inpatient Acute Case Management
- Coordinate care and transitions throughout the acute care patient experience.
- Collaborate with physicians, medical social workers, and bedside RNs.
- Ensure timely progression and transition of patients to prevent unnecessary admissions or readmissions.
- Communicate effectively and facilitate care across the continuum.
- Develop cost-effective care coordination plans.
- Identify and refer quality and risk management concerns.
- Conduct patient assessments and develop care plans.
- Manage utilization and ensure appropriate level of care.
- Formulate and implement transition plans.
- Address complex clinical and social situations to avoid unnecessary admissions.
- Participate in department operations and improve practices.
- Communicate effectively with colleagues and community agencies.
Salary : $74 - $76
