Position Overview

Talbot Health Services is seeking a skilled, dependable, and compassionate Licensed Practical Nurse (LPN) to join our behavioral health and substance use treatment team in Toledo, Ohio at our Monroe street site (5085 Monroe street)

This role is ideal for an LPN who is clinically strong, organized, patient-centered, and comfortable working in a fast-paced treatment environment. The LPN will support daily nursing operations, medication services, patient monitoring, documentation, and coordination of care for individuals receiving behavioral health and substance use disorder treatment services.

Key Responsibilities

The Licensed Practical Nurse will be responsible for providing high-quality nursing support in accordance with applicable laws, clinical standards, provider direction, and organizational policy. Responsibilities include, but are not limited to:

  • Administer medications and treatments as ordered and within LPN scope of practice.
  • Complete nursing observations, collect vital signs, and monitor patient status.
  • Identify and communicate changes in patient condition to appropriate clinical or medical staff.
  • Support medication-assisted treatment and other behavioral health services as applicable.
  • Assist with patient admissions, intakes, discharges, transfers, and ongoing care coordination.
  • Maintain accurate, timely, and professional documentation in the electronic health record.
  • Educate patients regarding medications, health concerns, treatment expectations, and wellness needs.
  • Coordinate with physicians, nurse practitioners, registered nurses, counselors, case managers, and leadership.
  • Assist with medication inventory, clinical supplies, lab coordination, and required nursing logs.
  • Follow infection control, safety, emergency response, and confidentiality procedures.
  • Maintain compliance with HIPAA, 42 CFR Part 2, and applicable behavioral health regulations.
  • Participate in staff meetings, trainings, quality improvement activities, and compliance initiatives.
  • Promote a respectful, trauma-informed, recovery-focused treatment environment.

Required Qualifications

  • Active Licensed Practical Nurse license in the State of Ohio.
  • Graduate of an accredited practical nursing program.
  • Current CPR/BLS certification, or ability to obtain upon hire.
  • Ability to complete required background checks and pre-employment screenings.
  • Strong communication, documentation, and patient care skills.
  • Basic computer proficiency and ability to use electronic health record systems.

Preferred Qualifications

  • Experience in behavioral health, addiction treatment, mental health, MAT, residential treatment, outpatient healthcare, or community-based care.
  • Familiarity with trauma-informed care, recovery principles, and patient-centered treatment.
  • Experience working with diverse patient populations.
  • Prior experience in a regulated healthcare or behavioral health environment.

Work Environment

This position works directly within a behavioral health and substance use treatment setting. The LPN may interact with patients in various stages of treatment, recovery, crisis, stabilization, and ongoing care. The role requires sound judgment, professionalism, compassion, and the ability to remain composed while supporting patient safety and clinical quality.
