Reports To: Clinical Supervisor
About the Role
Talbot Health Services is seeking a Case Manager to support clients participating in mental health and substance use disorder treatment. This position focuses on engagement, coordination, and ensuring each client can access the services necessary for their recovery.
Case Managers work closely with clinical staff, medical providers, and community partners to reduce barriers, improve stability, and promote whole-person wellness.
What You’ll Do
Provide person-centered case management aligned with each client’s individualized recovery plan
Monitor behavioral health needs, physical health concerns, and social determinants of health
Coordinate referrals to housing programs, benefits, medical providers, employment resources, and recovery supports
Assist with scheduling, appointment reminders, and follow-up to support attendance and engagement
Facilitate or assist in psychoeducational and skills groups as assigned
Complete documentation accurately and within required timeframes
Conduct outreach within 24 hours of receiving new cases and remain in regular contact based on client need
Participate in interdisciplinary treatment planning, care coordination, and transition planning
Maintain productivity expectations and comply with all agency policies and ethical standards
Required Qualifications
Active Ohio CDCA or ability to obtain within 90 days
High school diploma or GED
At least one year of experience in behavioral health, substance use treatment, or recovery support
CPR/First Aid/AED training (or willingness to complete within 30 days)
Crisis prevention/de-escalation training (or willingness to complete within 90 days)
Preferred Qualifications
Associate or bachelor’s degree in social services, psychology, or a related field
Experience working with co-occurring mental health and substance use disorders
Knowledge of community resources, benefits systems, and referral processes

Office-based with occasional community-based coordination
Frequent interaction with individuals experiencing acute symptoms or crises
Drug- and alcohol-free workplace
May involve local travel between Talbot sites
