Social Worker / Geriatric Care Manager (MSW/LMSW)

We are seeking a compassionate, organized, and self-motivated Social Worker / Geriatric Care Manager to join our growing team. This is an excellent opportunity for a social worker who is passionate about working with older adults, enjoys building meaningful relationships, working independently in the field, and collaborating with an experienced and supportive team
Key Responsibilities
- Manage a caseload of older adult clients (low 20s max)
- Conduct in-home visits and ongoing client assessments
- Develop and monitor individualized care plans
- Advocate for clients across healthcare, housing, and social service systems
- Assist with access to benefits, entitlements, and community resources
- Collaborate with families, providers, and support networks
- Maintain accurate and timely documentation
- Participate in weekly team meetings and case conferences
Required Qualifications
- LMSW or LCSW preferred (MSW considered with relevant experience)
- Experience in care management, case management, or working with older adults
- Valid driver’s license and reliable vehicle required
- Ability to conduct field-based home visits throughout NYC and surrounding areas
- Strong communication, organization, and documentation skills

Work Environment
