Role Description
This is a full-time, on-site role for a Clinical Case Manager based in Miami, FL. The Clinical Case Manager will be responsible for providing case management and social work services to clients, ensuring they receive the appropriate support and resources. Key responsibilities include conducting needs assessments, coordinating care plans, advocating for client needs, and maintaining accurate and confidential documentation. Additionally, the Clinical Case Manager will collaborate with professionals from mental health and social service backgrounds to enhance client outcomes and ensure comprehensive support.
Qualifications
- Expertise in Social Work, Case Management, and delivery of Case Management Services
- Strong knowledge and experience in Mental Health and Social Services
- Proven ability to assess client needs and create effective care plans
- Exceptional interpersonal, communication, and organizational skills
- Ability to work effectively within a team and across multidisciplinary networks
- Bachelor’s or Master’s degree in Social Work, Counseling, or a related field preferred
