Registered Nurse (RN)

Responsibilities:
Oversee and coordinate the delivery of quality care in accordance with each client's individualized care plan and needs.
Conduct home visits with clients within designated geographic territories.
Monitor clients' conditions and communicate any changes or concerns to the appropriate Clinical or Client Services Manager.
Review, follow up on, and properly document physician orders and changes in the plan of care.
Perform comprehensive client assessments and reassessments as necessary.
Coordinate care among clients, caregivers, families, physicians, and other members of the healthcare team.
Provide ongoing guidance and support to caregivers to ensure care is delivered in accordance with the client's care plan and agency standards.
Monitor the quality and continuity of services provided to clients.
Accurately document observations, interventions, assessments, follow-ups, and evaluations pertaining to client care and services provided.
Identify client care needs and assist in developing appropriate solutions to ensure the highest level of service and client satisfaction.
Qualifications:
Graduation from an accredited and approved nursing program.
Current and unrestricted Registered Nurse (RN) license in the State of New Jersey.
Previous experience in home care, community-based healthcare, or a similar setting, with a strong understanding of the unique needs and responsibilities involved in providing care in the client's home.
Strong clinical assessment, communication, organization, and care coordination skills.
Ability to work independently, make sound clinical decisions, and effectively coordinate client care.
Ability to communicate effectively with clients, families, caregivers, physicians, and members of the healthcare team.
