Job Title: Program Manager – Jon W. Paulson Veterans Community

Location: 1030 Girard Road, San Francisco, CA 94129 (Presidio)

Schedule: Monday – Friday, 8am-4pm or 9am-5pm

Reporting Type: In-Person

Employment Type: Exempt – Full-Time

Salary: $86,112/year


Summary:
The Program Manager, under the supervision of the Assistant Clinical Director of Residential Programs, will oversee Swords to Plowshares’ Permanent Supportive Housing’s supportive services program at the Jon W. Paulson Veterans Community. The duties of this position include direct client case management, the provision of a robust supportive services program, and the safety and well-being of residents. The Program Manager is also responsible for implementing direct services in an effort to prevent a return to homelessness through the provision of needed support such as substance abuse and mental health services. The Program Manager maintains an assigned caseload of veterans with various disabling conditions and provides individual counseling, group counseling, case management, assessment, advocacy, outreach, and crisis intervention. The Program Manager supervises a staff of Mental Health Specialists and a Peer Specialist and coordinates operations of the site with Property Management.


Responsibilities:

Administration:

  • Provide weekly supervision for a team of five staff
  • Pr ovide day-to-day support to the supportive services team to ensure high quality groups and activities are delivered to veteran residents throughout the month
  • Ensures programmatic, legal, and contract compliance, including timely and accurate reporting in order to achieve programmatic service and outcome objectives
  • Audits charts and physical files in compliance with agency requirements
  • Provides routine case conference facilitation with the supportive services team
  • Provides oversight and review of case management, clinical, and peer related service documentation of the team
  • Works in collaboration with the Program Manager for Residential Services to ensure the coordination of services and activities taking place on and offsite
  • Works in collaboration with Property Management to interview and intake new residents to the property
  • Performs census management in partnership with Property Management
  • Coordinates with property management and the supportive services team to ensure the teams are always working toward the goal of supporting housing stability and community building
  • Attend all required training to ensure individual and professional growth
  • Maintain compliance with Swords to Plowshares’ policy in addition to all federal, state, and local laws and regulations that govern our work
  • Other duties as assigned


Case Management:

  • Provide individual counseling for veterans with mental health and substance use treatment needs, including outreach and advocacy for an assigned caseload
  • Ensures coordination of treatment and referrals with other providers, such as the VA and other medical and social services providers in San Francisco
  • Maintain appropriate and timely documentation, including progress notes and Life-Plans to monitor progress towards the achievement of goals
  • Crisis intervention/de-escalation and conflict resolution
  • Engaging veterans to promote personal growth through onsite groups and activities
  • Participate in weekly supervision


Community Engagement:

  • Coordinate with Property Management to hold a monthly community meeting with residents.
  • Remain abreast of activities, resources, and services taking place within the Presidio and neighboring communities
  • Acts as a liaison with community programs within the Presidio such as the Institute on Aging and YMCA to support resident wellbeing
  • Develop and maintain an effective interface with community resources for advocacy and linkages to services assisting tenants in achieving goals
  • Participates in meetings required by funders and the agency


Supervisory Responsibilities (If Applicable)

Supervise four (4) on-site Mental Health Specialist and one (1) Peer Support staff


Desired Qualifications:

Experience:

  • Minimum of two (2) years' experience providing mental health, substance abuse counseling and case management services to disadvantaged or otherwise hard to reach clients preferably in a community based organization.
  • Experience in program management and/or staff supervision
  • Excellent time management skills
  • A thorough understanding of the needs of people experiencing homelessness, to include chronic homelessness with an emphasis on the readjustment difficulties of veterans, Post Traumatic Stress Disorder, substance abuse-related disorders and other mental health conditions
  • Ability to outreach and engage tenants
  • Computer skills and the ability to input progress notes and data, basic typing skills, Microsoft applications (e.g., Excel, PowerPoint, Word, Excel, and Teams.)
  • Understanding of confidentiality requirements
  • Good verbal communication skills
  • Crisis intervention, de-escalation, and conflict resolution skills
  • Ability to discharge duties in a professional manner
  • Ability to demonstrate clear professional boundaries
  • The demonstrated ability to work as part of a team


Education:

  • Preferred: Master's degree in Social work, Psychology, Counseling, or other related social service field
  • Experience may be substituted for education
