What You'll Do
As an LPN, you'll:
- Administer medications and treatments according to provider orders
- Monitor residents and respond to changes in condition
- Provide nursing care for chronic conditions and cognitive impairment
- Maintain accurate clinical documentation and medication records
- Support individualized resident service plans
- Communicate clinical concerns to nursing leadership and providers
- Provide clinical guidance to caregivers and medication staff within your scope
- Communicate with residents and families
- Coordinate with physicians, pharmacies, home health, hospice, therapy, and other healthcare partners
- Promote resident safety, dignity, independence, and quality of life
Bring the Nursing Experience You've Already Built
You'll need an active Minnesota Licensed Practical Nurse (LPN) license, sound clinical judgment, dependable communication skills, and confidence practicing within your scope.
Experience in Long-Term Care, Skilled Nursing, Rehabilitation, Assisted Living, Memory Care, Home Health, Hospice, Clinic Nursing, or Geriatric Care can translate especially well.
We're looking for an LPN who wants to participate in the care, communicate what they're seeing, and be someone the team can rely on clinically.
We're Recruiting on Both Sides of the Minnesota-Iowa Border
Elmore sits near the Minnesota-Iowa border, so we're interested in nurses throughout southern Minnesota and northern Iowa who can reasonably commute to the community.
On the Minnesota side, that includes Blue Earth, Winnebago, Fairmont, East Chain, Frost, Bricelyn, and surrounding communities.
On the Iowa side, we're also interested in nurses around Buffalo Center, Lakota, Ledyard, Swea City, Bancroft, Lake Mills, Forest City, Estherville, and surrounding northern Iowa communities.
Iowa-based applicants will need to be able to meet the Minnesota LPN licensure requirement for this position.
