SummitWest Care is a local nonprofit home health agency seeking a compassionate Physical Therapist Assistant (PTA) to provide one-on-one therapy services throughout Mesa County under the direction of a licensed Physical Therapist.

- Provide skilled therapy services in patient homes
- Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention
- Monitor and communicate patient progress and changes to supervising PT
- Encourage patient participation and reinforce goals
- Complete timely, accurate documentation in EMR
- Collaborate with therapists, nurses, caregivers, and other care team members
- Current Colorado Physical Therapist Assistant (PTA) license in good standing
- Graduate of an accredited Physical Therapist Assistant program
- Valid driver's license, reliable transportation, and auto insurance
- Strong communication and documentation skills
- Ability to work independently and adapt professionally in home-based setting
- Flexible, full-time or part-time scheduling
