- Evaluate patients and develop individualized, person-centered care plans
- Provide skilled physical therapy services in patient homes
- Improve patient mobility, strength, balance, and functional independence
- Educate patients and caregivers on treatment goals and home exercise programs
- Monitor patient progress and modify treatment plans to support individual goals
- Complete accurate and timely clinical documentation
- Collaborate with nurses, therapists, physicians, caregivers, and other members of the care team to achieve the best possible outcomes
- **Current Colorado Physical Therapist (PT) license** in good standing
- Graduate of an **accredited Physical Therapy program**
- Valid **driver's license, reliable transportation, and auto insurance**
- Strong **clinical assessment and treatment** skills
- Excellent **communication and documentation** abilities
- Ability to **work independently** while collaborating effectively with an interdisciplinary team
- Ability to **adapt professionally** in a home-based care environment
