What are the responsibilities and job description for the Patient Care Coordinator position at Summit Home Care?
Summit Home Care is seeking an experienced Patient Care Coordinator in our Valley Stream office. Patient Care Coordinators are responsible for staffing patient cases with appropriately matched home health aides, providing basic case management services and are responsible for maintaining communication with patients, office personnel, paraprofessional staff, and contract agencies.

Position Qualifications:
- Experienced patient care coordinator with a minimum of at least one year of experience in home care coordination.
- Must speak FLUENT English and Spanish. Really, if you don't speak BOTH FLUENT English and FLUENT Spanish, DO NOT APPLY. We aren't going to make an exception and nothing on your resume is going to impress us enough to change that.
- Excellent telephone and communication skills
- Excellent organizational skills
- Ability to maintain accurate schedules, notes and records
