The RN will provide high-quality patient care to residents with chronic illnesses, disabilities, and age-related conditions. You will collaborate with a team of healthcare professionals to ensure the physical, emotional, and social well-being of residents. Key Responsibilities:Conduct comprehensive assessments of residents’ health status, document and monitor changes in condition.Administer medications and treatments as prescribed.Provide direct patient care, including wound care, mobility assistance, and managing daily activities.Collaborate with physicians, therapists, and other healthcare team members to develop and implement care plans.Educate residents and families about health conditions, treatment options, and care techniques.Monitor and document patient progress and responses to treatments.Ensure compliance with all healthcare regulations and facility policies.Participate in care team meetings and contribute to the overall care management process.
