The RN will provide high-quality, patient-centered care in patients' homes, assisting with recovery, chronic disease management, and maintaining overall health. This role requires excellent assessment skills, the ability to work independently, and a commitment to improving patient outcomes in a home setting. Responsibilities:Conduct comprehensive patient assessments, including physical, emotional, and social well-being.Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.Administer medications, treatments, and procedures as prescribed, including wound care and IV therapy.Monitor patient progress, vital signs, and respond to any changes in condition.Provide education to patients and families on disease management, medications, and self-care techniques.Coordinate care with physicians, therapists, and other healthcare professionals.Document all patient interactions, treatments, and progress in the electronic health record.
