The Sleep Nurse Navigator serves as a clinical care coordinator and patient advocate within the Sleep Medicine program. This role helps navigate patients through diagnostic testing procedures, treatment initiation/activation and follow-up management for sleep disorders, including obstructive sleep apnea, insomnia, restless legs syndrome, and other sleep-related conditions. The Nurse Navigator collaborates closely with Sleep Medicine care team, ENT care team, Registered Polysomnography Technologists, primary care teams, and durable medical equipment (DME) providers to optimize patient outcomes and support long term adherence to treatment. The Sleep Nurse Navigator is essential in providing extensive patient education and personalized care coordination leading to improved patient outcomes, enhancing sleep therapy adherence, and contributes to the overall effectiveness of the Sleep Medicine program.

Education Qualifications
- Bachelor's of Science in Nursing (BSN) Preferred
- 2 years Professional Nursing experience required, preferably in sleep medicine, pulmonary, ENT, neurology or other related specialty. Required
- Prior patient education roles and case management preferred Preferred
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to read, analyzes, and interprets general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to effectively present information and respond to questions from physicians, patients, co-workers and the general public.
- Professional nursing theory and practice to give and evaluate patient care:
- Medical equipment and instruments to administer patient care

- Ability to define problems collects data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in the mathematical diagram form and deal with several abstracts and concrete variables.

- Basic Life Support required within 90 days of hire
- Serves as the primary point of contact for patients navigating the sleep care continuum—from referral through diagnosis, treatment initiation, and ongoing follow up to enhance patient satisfaction and compliance.
- For Hypoglossal Nerve Stimulator patients, works directly with the Sleep Medicine care team, ENT care team, Sleep center care team and other team members to coordinate diagnostic tests, procedures and follow up appointments, ensuring patients remains on the patient pathway.
- Provides extensive education to Hypoglossal Nerve Stimulator patients from initial interest in device and procedure to activation and follow up appts.
- Collaborate with others to increase awareness among referral sources of services provided by the Sleep Medicine team to drive program growth.
- Maintain accurate documentation within the electronic health record (EHR), including patient encounters, care plans and education provided to the patient.
- Collects and maintains data to track patient information, pathway compliance and outcomes and presents data at quarterly administrative/provider meetings.
- Provide individualized education on sleep disorder diagnosis, CPAP therapy, mask fitting, and lifestyle modifications as needed in the Sleep Medicine Clinic.
- Work with provider on activations, titrating settings as needed for patient.
- Write and maintain standard work, policies and procedures.
- Evaluate workflows and continually seek opportunities to streamline the pathway.
- Participate in sleep medicine clinic performance improvement projects and center of excellence accreditation preparation.
- Maintain current knowledge of sleep medicine best practices and new therapies/treatments.
- Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
