We are seeking a Physical Therapist (PT) to join a rehabilitation team on a PRN basis in Greenfield, MA. This flexible opportunity allows you to provide individualized therapy while helping residents improve mobility, strength, balance, and overall function.
What You'll Do

• Perform physical therapy evaluations and develop individualized plans of care
• Provide skilled interventions addressing gait, strength, balance, transfers, endurance, and mobility
• Establish functional goals and monitor resident progress
• Adjust treatment approaches based on clinical findings and resident needs
• Educate residents and caregivers on exercises and safe mobility strategies
• Maintain accurate and timely clinical documentation
• Coordinate care with therapy, nursing, and other healthcare professionals

Qualifications

• Active Massachusetts Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical and communication skills
• Skilled nursing experience preferred, but not required
• New graduate Physical Therapists are encouraged to apply
