Help residents move toward their next recovery milestone. Stern Therapy Consultants is seeking a PRN Physical Therapist at Autumn Lake at Salem County, providing skilled nursing rehabilitation that connects clinical treatment with residents' everyday mobility needs.
What You'll Do
Evaluate mobility, strength, balance, and range of motion while considering medical precautions and prior function.
Develop individualized plans of care with measurable goals for walking, transfers, and daily movement.
Provide therapeutic exercise, gait training, and functional mobility treatment appropriate to each resident.
Assess assistive device needs and educate residents and caregivers on safe movement and fall prevention.
Reassess progress, adjust interventions, and supervise PTA services within the treatment plan.
Document clinical findings and outcomes, coordinating recommendations with nursing and the rehabilitation team for ongoing care and discharge.
