Job Responsibilities
- Participate in the admission, discharge, and transfer of residents, monitoring changes in condition and documenting appropriately
- Transcribe and carry out physician orders
- Administer medications and treatments in accordance with physician orders and facility policies
- Perform wound care and other nursing treatments as directed
- Provide direct nursing care, including tube feedings, colostomy care, and IV administration as applicable
- Provide guidance and leadership to nursing assistants and other nursing personnel
- Collaborate with the interdisciplinary care team to ensure residents' physical, emotional, and clinical needs are met
- Initiate, review, and update resident care plans as required
- Monitor residents for changes in condition and report concerns promptly
- Treat all residents with dignity, respect, and compassion while promoting their rights and independence
- Follow all facility policies, procedures, and applicable regulations
- Maintain confidentiality of resident and protected health information
- Report concerns related to noncompliance, retaliation, discrimination, or resident safety to appropriate leadership
- Participate in emergency preparedness, life-safety drills, and required training
- Maintain accurate and timely clinical documentation
- Current, valid LPN license in the state of Vermont
- Current CPR certification
- At least 1 year of nursing experience preferred
- Long-term care, skilled nursing, or gerontological nursing experience preferred
- Strong clinical, communication, organizational, and documentation skills
- Ability to work effectively as part of an interdisciplinary team
- $2,000 per week
