Responsibilities
- Provide speech therapy services that promote excellence in rehabilitation care and quality patient outcomes.
- Promote interdisciplinary cooperation and support the facility's geriatric rehabilitation program.
- Evaluate patients in a timely manner according to state, payer, and facility requirements.
- Screen patients upon admission and/or when requested by the Director.
- Develop individualized treatment plans and appropriate goals based on patient evaluations.
- Provide treatment according to the established plan of care.
- Communicate and collaborate with other disciplines and staff as indicated by the treatment plan.
- Provide routine documentation in accordance with Medicare, facility, and other payer guidelines.
- Participate in family conferences when requested.
- Record and submit all required documentation and treatment charges in a timely manner according to facility policies and procedures.
- Current Vermont State License as a Speech-Language Pathologist.
- Ability to work effectively as part of a dynamic and cooperative interdisciplinary team.
- Strong assessment and evaluation skills.
- Ability to develop individualized treatment plans and appropriate discharge plans.
- Ability to participate in consultations, teaching, and meetings with residents, families, staff, and other healthcare professionals.
- Strong communication and interpersonal skills.
- Ability to educate and counsel patients, families, and staff as appropriate.
