Responsibilities:
- Assess patients' medical conditions, functional capabilities, limitations, and restrictions, and establish a treatment program with specific goals under the direction of the Physician.
- Provide direct patient care, set realistic and achievable goals, and document and record patients' conditions and educate patients and families.
- Collaborate with all disciplines to provide best patient-centered treatment and care.
- Perform all other duties assigned by the Director of Rehabilitation.
- Must have graduated from an accredited Physical Therapy college or university.
- Must have a valid professional license and registration for the state of practice.
- Experience with long-term care and/or geriatric care is preferred but not limited to.
