Responsibilities:
- Conduct and coordinate the development and completion of resident assessments
- Conduct resident assessments in accordance with current rules, regulations, and guidelines
- Implement resident Care Area Assessments (CAAs)
- Work collaboratively with the interdisciplinary care team to develop comprehensive assessments and individualized care plans for each resident
- Participate in facility survey readiness activities and support the facility throughout the survey process
- Ensure assessment and care planning processes are completed accurately and in accordance with applicable requirements
- Collaborate with nursing staff and other healthcare professionals to support quality resident outcomes
- Current, unencumbered Vermont RN or LPN license
- Minimum of two years of MDS and PPS experience
- Previous care planning experience
- Strong organizational, communication, and assessment skills
- Ability to work effectively with an interdisciplinary healthcare team
- Commitment to providing high-quality, resident-centered care
