MDS Coordinator

FULL TIME

Qualifications for MDS Nurse:

• 
  Current unencumbered State of Massachusetts RN or LPN License

• 
  Two years of MDS and PDPMexperience

• 
  Care Planning experience

Responsibilities for MDS Nurse:

• 
  Conduct and coordinate the development and completion of the resident assessment
• 
  Conduct resident assessments in accordance with current rules, regulation, and guidelines
• 
  Implementation of resident CAA’s
• 
  Participate in facility survey readiness and throughout survey

• 
  Work with our interdisciplinary care team min developing a comprehensive assessment and care plan for each resident.
