Responsibilities
- Coordinate the development and completion of comprehensive resident assessments
- Complete MDS assessments in accordance with current state and federal regulations
- Conduct and coordinate Care Area Assessments (CAAs)
- Coordinate and support individualized resident care plans
- Collaborate with nursing, therapy, social services, dietary, physicians, and other members of the interdisciplinary team
- Ensure MDS documentation is accurate, complete, and submitted within required timelines
- Monitor MDS schedules and regulatory deadlines
- Participate in facility survey preparation and the survey process
- Review resident documentation for accuracy and completeness
- Communicate effectively with residents, families, and interdisciplinary team members
- Support quality improvement and regulatory compliance initiatives
- Current, unencumbered Vermont RN or LPN license
- Previous MDS and PPS experience required
- Previous care planning experience preferred
- Strong knowledge of MDS, PPS, and applicable regulatory requirements
- Excellent assessment, documentation, organizational, and communication skills
- Strong attention to detail and ability to meet deadlines
- Ability to work effectively as part of an interdisciplinary team
