Responsibilities
- Direct the daily activities of licensed practical nurses and certified nursing assistants in accordance with current federal, state, and local regulations, guidelines, and facility policies
- Follow the nursing model to promote holistic, resident-centered care
- Ensure nursing personnel comply with facility policies and procedures
- Participate in resident admissions, discharges, and transfers
- Assess residents for changes in condition, notify physicians and families or representatives, and document accordingly
- Transcribe physician orders and carry out orders as written
- Prepare and administer medications as ordered and monitor for adverse effects
- Perform wound treatments as ordered and document changes appropriately
- Schedule follow-up appointments and coordinate transportation needs
- Provide nursing leadership and guidance to nursing personnel assigned to the unit
- Provide direct care, including colostomy changes, tube feedings, wound care, suctioning, IV administration, and other skilled nursing services as assigned
- Conduct rounds to ensure resident needs are being met and staff are performing their assigned duties
- Collaborate with interdisciplinary team members to ensure residents' needs are fully addressed
- Participate in management team meetings to discuss census changes, resident status, care needs, complaints, and concerns
- Assist with evaluating nursing staff performance and implement disciplinary action according to facility policies and direction
- Current, unrestricted Vermont RN or LPN license
- Nursing degree from an accredited college or university
- Current CPR certification
- Prior long-term care experience preferred but not required
- Strong leadership, communication, and critical-thinking skills
- Ability to work effectively in a fast-paced healthcare environment
