Responsibilities:
- Supervise Certified Nursing Assistant (CNA) staff on assigned units
- Deliver care in accordance with Facility protocol and safe nursing practices
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Provide direct patient care, including administering medications, wound care, and other treatments as needed
- Maintain accurate and detailed records of patient care and progress
- Current unencumbered State of Vermont LPN license
- Strong communication skills to communicate with residents, families, and staff
- Critical thinking skills to problem-solve and make informed decisions
- Ability to handle different situations with tact and professionalism
- High school diploma or equivalent required
