One of ten districts across Louisiana, MHSD was created by the state legislature in 2003 to oversee the delivery of publicly funded, community-based mental health, addictive disorders and developmental disabilities services to our area. MHSD serves adults, children and families of Orleans, Plaquemines and St. Bernard Parishes. In particular, we serve residents who are uninsured, or Medicaid eligible/covered, individuals suffering from mental illness, individuals suffering from addiction, and individuals who have developmental and/or intellectual disabilities.
We are currently hiring for a Community Service Professional 3
The Community Service Professional 3 serves a critical role within the Metropolitan Human Services District (MHSD) Intellectual and Developmental Disabilities (IDD) program by providing person-centered case management and support coordination services to individuals with developmental disabilities and their families. This position is responsible for developing, implementing, and monitoring individualized Plans of Support; coordinating services and resources; advocating for individuals' needs; ensuring compliance with state and federal regulations; providing crisis intervention and case management; and collaborating with community partners, service providers, and families to promote health, safety, independence, and quality of life.

Six years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; ORSix years of full-time work experience plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
A bachelor's degree plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
An advanced degree in a social sciences field, nursing, public health; public, hospital, or health administration; speech-language pathology or communication sciences and disorders; physical, occupational, recreational, or music therapy plus two years of experience in providing health or developmentally disabled services, counseling, health care management, or social services.
EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

- Implements the Plan of Support by identifying natural and community resources that may provide the support needed. Submits referral of information to agencies for individuals requesting services outside of the Family Support services, such as, to EPSDT providers, regional Medicaid offices, OCDD LGE offices, etc. Assists family/individual in navigating the referral process to other agencies/resources.
- Advocates for individuals of the funding decisions on service requests by preparing the following: Family Support Decision Letter, Rights of Appeal Notice, Individual Family Support Agreement or Authorization Letter to contracted agency for funding, Financial Activity form, and required Individual Tracking forms to ensure that information is input into OCDD Statewide and MHSD/OCDD regional database
- Provide support coordination to individuals involved in the court system, to include completing court reports two days of the scheduled hearings, testifying as needed
- Implements crisis case management services including continuous follow-up to ensure service delivery and meets with the individual and/or family on daily or weekly basis until the crisis is resolved.
- Attends and participates in division and district staff meetings, trainings, sessions, as needed
